Hard-to-heal wounds don't stay clean on their own.
A wound is generally considered hard-to-heal when it hasn't closed by 40–50% after four weeks of good standard care. Most of these wounds carry a biofilm — a protective layer of bacteria and debris that blocks healing and can reform within 48–72 hours of being removed.
That's why debridement isn't a one-off step. It needs to happen regularly, as part of an ongoing wound care routine, to stop biofilm from re-establishing and to keep the wound bed, edges and surrounding skin progressing toward closure.
